PDO Threads Consent Form
This consent form is intended to confirm that you have been fully informed about PDO (polydioxanone) thread treatments, including their purpose, limitations, risks, benefits, alternatives, and expected outcomes. This information allows you to make an informed and voluntary decision to proceed.
General Understanding of PDO Threads
I understand that PDO (polydioxanone) threads are absorbable surgical sutures placed into the skin or subdermal tissue to stimulate collagen production, improve skin quality, and in some cases provide mild lifting or repositioning of superficial tissues.
I understand that PDO thread treatments are considered a minimally invasive aesthetic procedure and are not a replacement for surgical facelift procedures. I acknowledge that PDO threads are a cosmetic enhancement and a “perk” treatment, not a definitive surgical correction.
Types of PDO Threads & Treatment Differences
Smooth PDO Threads
- Placed primarily in the dermis
- Designed for skin tightening, texture improvement, and collagen stimulation
- Do not lift or reposition fat pads
- Typically require a series of treatments, usually 2–4 sessions, spaced 4–8 weeks apart for optimal results
Molded / Barbed PDO Threads (Lifting Threads)
- Placed in deeper tissue planes
- Designed to reposition superficial fat pads and soft tissue
- Provide structural support and lifting
- Typically last approximately 12 months or longer, depending on individual factors
- A follow-up or adjustment treatment may be necessary to improve balance or symmetry
I understand that PDO threads alone may not address all aspects of facial aging, and that I may still require dermal fillers for volume restoration, biostimulators for additional collagen support, or skin tightening or resurfacing treatments for skin quality.
Expected Results & Timeline
I understand that results are not immediate. Initial tightening may be noticed early, but collagen stimulation develops gradually.
I understand that optimal results typically appear 2–3 months after treatment, with continued improvement over time.
I acknowledge that maximal correction and collagen remodeling may take up to 12–18 months, depending on my biology and the type of threads used.
No Guarantee / Expectation Disclaimer
I understand and acknowledge that no guarantee, warranty, or assurance has been made regarding my results.
I understand that individual results vary based on age, genetics, skin quality, tissue laxity, medical history, lifestyle factors, and biological response.
I understand that personal or subjective dissatisfaction does not constitute a complication or treatment failure, and that aesthetic medicine is not an exact science.
I understand that additional treatments may be recommended and may involve additional cost.
Alternatives
I understand that alternatives to PDO threads may include dermal fillers, biostimulatory injectables, energy-based skin tightening, surgical procedures, or no treatment at all.
Medical Disclosure & Responsibilities
I have informed my practitioner of any previous dermal filler or biostimulator injections, including type, date, and area treated.
I will notify my provider of all medical conditions, allergies, medications, supplements, and prior procedures.
I deny the possibility of being pregnant, trying to become pregnant, or breastfeeding. I understand that PDO thread safety has not been established during pregnancy or breastfeeding.
Risks & Possible Complications
I understand that risks may include discomfort, tenderness, or tightness; bruising, swelling, or redness; bleeding or hematoma; infection; asymmetry or imbalance; thread visibility, palpability, or extrusion; skin dimpling or puckering; pigment changes; scarring at entry points; nerve or vessel irritation (rare); and the need for thread trimming, adjustment, or removal.
I understand that thread extrusion may occur, requiring trimming or removal, and that follow-up treatments may be necessary to optimize balance or outcome.
Post-Treatment Responsibility
I understand it is my responsibility to notify AI Wellness Center of Excellence promptly of any concerns, complications, or unusual symptoms and to allow my provider the opportunity to evaluate and manage care.
I understand that seeking outside care without notifying my treating provider may interfere with proper management.
Authorization & Consent
The procedure, including benefits, risks, alternatives, and expected outcomes, has been fully explained to me.
I have had the opportunity to ask questions and all questions have been answered to my satisfaction.
I authorize Dr. Gideon Kwok, D.O., and/or qualified medical associates at AI Wellness Center of Excellence to perform PDO thread procedures on me.
I acknowledge that payments are for the service performed, not for a guaranteed outcome, and that there are no refunds for patient-determined unsatisfactory results.
Final Consent
I certify that I have read, understand, and voluntarily agree to this PDO Threads Consent. I understand that this document supersedes all prior verbal or written disclosures.
