AI Wellness Membership Terms & Conditions
Applicable to All Membership Types:
- Platinum Longevity Membership (Individual)
- Platinum Brand Partner Membership (Brand / Provider)
- Platinum Location Partner Membership (Clinic / Medspa / Hotel / Venue)
1. Membership Eligibility
1.1 All individual memberships are open to persons 18 years of age or older.
1.2 Brand Partner and Location Partner memberships are open to legally registered businesses, organizations, and licensed operators in good standing.
1.3 By enrolling, you confirm that all information provided during registration is accurate, complete, and up to date.
1.4 AI Wellness Center of Excellence reserves the right to approve, deny, suspend, or revoke any membership if eligibility requirements are not met or if a member fails to comply with these Terms & Conditions.
2. Payment & Billing Policy
2.1 Membership investment options by type:

2.2 Membership activation requires successful payment processing.
2.3 All membership fees are non-refundable once the membership is activated.
2.4 If a monthly payment fails or becomes delinquent, membership benefits may be suspended until payment is resolved.
2.5 AI Wellness reserves the right to adjust membership pricing for future enrollments or renewals.
2.6 Brand Partner and Location Partner payment plans, financing terms, and tier pricing are subject to a signed partnership agreement.
3. Membership Term & Renewal
3.1 All membership types carry a 12-month commitment unless otherwise specified in a signed agreement.
3.2 Membership benefits begin on the earlier of:
- the member's first treatment or first activation of partner benefits, or
- 30 days after enrollment
3.3 Memberships may automatically renew annually unless cancellation is requested at least 30 days prior to the renewal date.
3.4 If membership is not renewed, access to all membership services and benefits will expire at the end of the membership term.
4. Appointment Policy
(Applicable to Platinum Longevity — Individual Members)
4.1 Appointments must be scheduled in advance and are subject to availability.
4.2 A minimum of 24 hours notice is required for appointment cancellation or rescheduling.
4.3 Missed appointments or cancellations with less than 24 hours notice may result in the deduction of a membership treatment credit.
5. Membership Freeze Policy
(Applicable to Platinum Longevity — Individual Members)
5.1 Members may request one membership freeze per year for up to 30 days.
5.2 Freeze requests must be submitted in advance and approved by AI Wellness management.
5.3 Membership benefits may be paused during the freeze period and the membership term may be extended accordingly.
5.4 Brand Partner and Location Partner membership freeze requests are evaluated on a case-by-case basis and must be submitted in writing to AI Wellness management for approval.
6. Early Cancellation Policy
6.1 All membership types require a 12-month financial commitment.
6.2 If a member or partner cancels before the end of the membership term, the remaining balance of the membership agreement becomes immediately due.
6.3 Early cancellation does not entitle the member or partner to any refund of fees paid.
6.4 Brand Partner and Location Partner cancellations must be submitted in writing and are subject to the terms outlined in their respective signed partnership agreements.
7. Non-Transferability
7.1 All membership benefits, services, treatment credits, partner privileges, and ecosystem access are non-transferable.
7.2 Platinum Longevity membership services may only be used by the registered individual member and may not be shared, gifted, or transferred to another person.
7.3 Brand Partner and Location Partner memberships are tied to the registered business entity and may not be transferred, sublicensed, or assigned to another organization without prior written approval from AI Wellness.
8. Partner-Specific Terms
(Applicable to Platinum Brand Partner & Platinum Location Partner Members)
8.1 Brand Partner and Location Partner memberships are governed by a separate signed partnership agreement in addition to these Terms & Conditions.
8.2 Access to the AI Wellness Platinum Network, brand promotion, event participation, marketplace inclusion, and financing programs is contingent upon the partner's membership remaining active and in good standing.
8.3 AI Wellness in-house financing access extended to a partner's clients is subject to the financing program's own terms, eligibility requirements, and approval process. AI Wellness does not guarantee financing approval for any individual client.
8.4 Location Partners are responsible for ensuring that all AI Wellness programs, technologies, and brand activations deployed within their location comply with applicable local, state, and federal laws and regulations.
8.5 Brand Partners are responsible for ensuring that all products and services promoted through the AI Wellness ecosystem are accurately represented, legally compliant, and meet the quality standards required by AI Wellness.
8.6 AI Wellness reserves the right to remove a brand, product, service, or location from the Platinum Network if it is found to be in violation of network standards, ethical guidelines, or these Terms & Conditions.
9. Member & Partner Conduct
9.1 All members and partners agree to follow AI Wellness clinic policies, staff instructions, and safety guidelines while using or operating within AI Wellness services and facilities.
9.2 Disruptive, abusive, fraudulent, misrepresentative, or unsafe behavior may result in immediate suspension or termination of membership or partnership.
9.3 AI Wellness reserves the right to deny services or revoke partnership access to any individual or organization that violates clinic policies or creates safety, legal, or reputational risks.
10. Liability & Medical Disclaimer
10.1 AI Wellness services and therapies are intended to support general wellness and preventative health.
10.2 Membership services do not replace professional medical care, emergency care, or treatment for acute medical conditions.
10.3 Members are encouraged to consult with their physician regarding any medical conditions or treatment decisions.
10.4 AI Wellness does not guarantee specific medical outcomes for any individual member.
10.5 Members and partners assume full responsibility for participation in treatments, wellness programs, and any activations conducted within or in association with the AI Wellness ecosystem.
10.6 AI Wellness is not liable for outcomes resulting from products or services provided by Brand Partners or activated by Location Partners within their respective facilities.
11. Data Privacy
11.1 AI Wellness may collect personal, biometric, health-related, and business data to support membership services, partner programs, and program personalization.
11.2 All data is handled in accordance with applicable privacy and healthcare data protection standards.
11.3 Brand Partners and Location Partners agree not to misuse, resell, or improperly disclose any member or patient data accessed through the AI Wellness ecosystem.
11.4 Members and partners may request updates or corrections to their information by contacting AI Wellness directly.
12. Governing Law & Dispute Resolution
12.1 These Terms & Conditions are governed by the laws of the State of California.
12.2 Any dispute arising under these terms should first be addressed through good-faith negotiation or mediation between the parties.
12.3 If unresolved, disputes may be resolved through binding arbitration or appropriate courts located in California.
13. Force Majeure
13.1 AI Wellness is not responsible for delays or inability to provide services due to events beyond its reasonable control, including but not limited to:
- Natural disasters
- Public health emergencies
- Government restrictions
- Equipment failure
- Power outages
- Other unforeseen circumstances
14. Agreement & Acceptance
By enrolling in any AI Wellness Platinum Membership — whether as an Individual Member, Brand Partner, or Location Partner — you acknowledge that you have read, understood, and agree to these Terms & Conditions.
Continued participation in any membership program constitutes acceptance of any future updates or modifications to these policies.
All Brand Partner and Location Partner memberships additionally require execution of a signed partnership agreement, which supplements and operates alongside these Terms & Conditions.
