Grant of Rights (Worldwide, Perpetual, Irrevocable)

For good and valuable consideration, I hereby grant to AI Wellness Center of Excellence Inc. (“Company”), its affiliates, partners, successors, assigns, and licensees the perpetual, worldwide, royalty-free, irrevocable right to:

• Record my voice, image, likeness, appearance, statements, performance, and participation during the podcast session(s).

• Use such recordings in audio, video, livestream, short-form content, long-form content, trailers, reels, ads, marketing materials, and all media formats.

• Edit, modify, adapt, mix, crop, enhance, or repurpose my appearance at their sole discretion.

• Use my name, likeness, professional information, biography, and voice in all promotional materials.

• Incorporate my appearance into other programs, including but not limited to:
  • Infinite Wellness: The New Human Era (Feature Film)

  • The AI Wellness Connect App

  • Infinity Interactive Directory

  • Masterclasses, teaching modules, conference materials

  • Smart Vending & Digital Screens

  • Onsite clinic displays

  • Social & digital advertising

This grant applies to all existing media and all future media yet to be invented.

Ownership

I acknowledge that all audio, video, photography, transcripts, and materials captured during the podcast are the exclusive property of AI Wellness Center of Excellence Inc.

I waive the right:

• to inspect or approve the final product

• to request changes, edits, or removal

• to restrict use of my appearance

License to Use Guest-Generated Content

If I create social media content before, during, or after my appearance, I grant Company the right to:

• Repost, share, feature, or repurpose my posts

• Use my likeness, handle, name, captions, and comments
  
• Use the influencer-style content for promotional or commercial use

No Compensation

I understand and agree:

• I am appearing voluntarily as a guest

• I am not entitled to monetary compensation

• I waive rights to royalties, revenue share, licensing payments, or future compensation

Any complimentary treatments, products, or experiences are in-kind only.

Consent for Video Recording

I understand the podcast session will be filmed and photographed as part of the Voice of Evidence Network and may appear in:

• Full podcast episodes

• Short clips for TikTok, Instagram, YouTube, Reels

• Trailers & promotional material

• Paid advertising

• Media kits, speaker reels, sizzle reels

• In-clinic displays and digital signage

Multiple Hosts & Additional Participants

I understand my episode may include:

• Multiple hosts

• Co-hosts

• Guest experts

• Influencers

• Panelists

• Live studio attendees
  

And I consent to appearing alongside them.

Future Appearances

This agreement applies to:

• Today’s episode

• Any future episodes

• Any pop-in guest appearances

• Any recurring guest roles or expanded features

No additional forms are required unless requested by Company.

Release of Liability

I hereby release and discharge:

• AI Wellness Center of Excellence Inc.

• AI Wellness Health Management Inc.

• Voice of Evidence hosts and producers

• Filmmakers, videographers, editors, media team

• All partners, affiliates, licensees, assignees
  

from any and all claims related to:

• Privacy

• Defamation

• Right of publicity

• Emotional distress

• Misrepresentation

• Editing decisions

• Commercial use

• Any distribution or exploitation of the recordings
  

This includes unknown and future claims.

No Revocation

This consent is irrevocable, perpetual, and may not be withdrawn.

Assignment

Company may assign or license this agreement to:

• Media partners

• Streaming platforms

• Distributors

• Film studios

• Advertising agencies
  

without additional approval.

Governing Law

This Agreement is governed by the laws of the State of California.
