Agreements, Disclaimers, Consents & Code of Conduct
1. Submission & Participation Acknowledgment
- I understand that my submission does not guarantee acceptance and must be reviewed and approved by AI Wellness before participation is finalized.
- I understand AI Wellness may contact me for additional information, interviews, or qualifications before confirming my participation.
- I acknowledge that all participation decisions are at the sole discretion of the AI Wellness curatorial committee and leadership team.
2. Media, Filming & Likeness Consent
- I consent to filming, photography, and interviews during AI Wellness events and programs.
- I grant AI Wellness the irrevocable right to use my name, likeness, practice name, brand, and submitted materials in global marketing, promotional, educational, and documentary content across digital and traditional platforms.
- I acknowledge that AI Wellness may amplify my participation through its official channels, including the Connect App, Pavilion screens, AI Wellness TV, Infinity Yearbook, and affiliated media outlets.
- I understand this content may be edited, repurposed, and distributed without additional approval.
3. Brand & Intellectual Property Representation
- I confirm that I am authorized to represent the brand, practice, or organization submitted.
- I grant AI Wellness a non-exclusive license to use brand names, logos, images, and submitted materials for promotional and educational purposes.
- I acknowledge that AI Wellness Avatars and CRM-powered experiences are educational and storytelling tools, not official sales or customer service channels.
4. Professional Credentials & Alignment
- I confirm that I am a licensed practitioner, wellness leader, or authorized representative of the brand I am submitting (where applicable).
- I acknowledge that I must operate in alignment with AI Wellness’ mission of forward-thinking, ethical, impact-driven wellness.
- I understand AI Wellness may require verification of my credentials, licenses, or practice before participation is confirmed.
5. Fees, Refunds & Transfers
- I acknowledge that all participation fees, registrations, and memberships are non-refundable once invoiced and confirmed.
- Transfers or rescheduling may be permitted only with prior written approval from AI Wellness.
- I understand upgrades such as the Infinity Yearbook are optional but may be required for certain ecosystem visibility opportunities.
6. Health, Wellness & Services Disclaimer
- I understand AI Wellness programs, avatars, and media do not provide personal medical advice, diagnosis, or treatment.
- I acknowledge that wellness therapies may be investigational and outcomes vary by individual.
- I agree that AI Wellness memberships and services do not replace health insurance or the care of a licensed primary care physician.
- I understand that all services delivered in Centers of Excellence or partner clinics are provided by licensed professionals.
7. Media & Press Conduct (Media Row / Accredited Press)
- I agree to uphold professional media ethics and not harm, defame, or disrupt the AI Wellness community.
- I understand Media Row access is limited and curated; final approval is required.
- I acknowledge there are limited Media Station slots, and top-tier access (e.g., Hermes Longevity Lounge) is invite-only.
- I understand that failure to comply with guidelines may result in immediate removal.
8. Code of Conduct
All participants, practitioners, partners, and media agree to:
- Uphold respectful, professional, and ethical behavior at all times.
- Avoid defamatory, harmful, or misleading statements about AI Wellness or fellow participants.
- Engage in forward-thinking, innovative, and impact-driven practices consistent with AI Wellness values.
- Respect the privacy and intellectual property of other participants, brands, and attendees.
- Comply with event staff directions, credentialing processes, and community guidelines.
- Violations may result in removal from events, termination of participation, or forfeiture of fees without refund.
9. Technology, Data & Privacy
- I understand AI Wellness CRM, avatars, and platforms are subject to evolving AI technology and third-party updates.
- I acknowledge that performance may vary depending on device compatibility, internet access, or platform changes.
- I consent to the secure storage of data collected through AI Wellness programs, avatars, or CRM systems, and understand that such data will only be used within the AI Wellness ecosystem unless otherwise agreed in writing.
- I authorize AI Wellness to securely share my pitch deck, revenue data, company profile, or submitted materials strictly for evaluation, investment, and educational purposes.
9A. Marketing Communications & Consent
- I consent to AI Wellness collecting, storing, and using my contact information (including name, email, phone, and submitted materials) for the purpose of providing program updates, event invitations, newsletters, marketing communications, and promotional offers related to the AI Wellness ecosystem.
- I understand that I may unsubscribe or opt out of marketing communications at any time by following the instructions provided in each email or contacting AI Wellness directly.
- I acknowledge that unsubscribing from marketing communications will not affect my receipt of essential participation or transactional communications related to my registration, membership, or obligations under these Terms.
10. Finance & Legal Acknowledgment
- I understand AI Wellness does not provide legal, tax, or investment advice.
- I acknowledge that any investment decisions are solely my responsibility and must comply with applicable laws.
- I acknowledge I may be contacted by the AI Wellness Finance Team (for payment and revenue-share onboarding) and the Legal Team (for contracts and compliance).
11. Non-Circumvention of Investors
- I agree not to bypass, circumvent, or attempt to independently contract with any investors, family offices, financial partners, or collaborators introduced to me through AI Wellness or The Pitch Room without written consent.
- Any such attempt shall still obligate me to honor the success fee terms below.
12. Success Fee & Revenue Share
- I understand AI Wellness Health Management, Inc. may charge a consulting/introducer fee for facilitating introductions.
- I agree that AI Wellness shall receive a 5% consultant success fee on the gross amount of any investment, funding, or deal closed as a result of introductions, opportunities, or facilitation by AI Wellness, The Pitch Room, Business Row, or affiliated ecosystem programs.
- This applies to all deals initiated or closed within 24 months of introductions.
- The 5% consultant success fee applies to cash, equity, convertible notes, SAFE agreements, revenue-share arrangements, and in-kind transactions.
- Payment is due within 10 business days of receipt of funds or finalization of equity/transactional agreements.
13. Confidentiality Obligation
- I understand AI Wellness Health Management, Inc. may charge a consulting/introducer fee for facilitating introductions.
- I agree that AI Wellness shall receive a 5% consultant success fee on the gross amount of any investment, funding, or deal closed as a result of introductions, opportunities, or facilitation by AI Wellness, The Pitch Room, Business Row, or affiliated ecosystem programs.
- This applies to all deals initiated or closed within 24 months of introductions.
- The 5% consultant success fee applies to cash, equity, convertible notes, SAFE agreements, revenue-share arrangements, and in-kind transactions.

14. Liability & Indemnification
- I release and hold harmless AI Wellness Health Management, Inc. from any claims, damages, or liabilities arising from my participation, my representations, or dealings with investors, brands, or third parties.
- Participation is at my own discretion; AI Wellness is not liable for business outcomes.
16. Escrow & Payment Handling
- I understand that certain transactions may be subject to escrow, compliance checks, or secure payment handling by AI Wellness finance partners.
17. Anti-Solicitation
- I agree not to solicit or pressure investors, media, or partners outside of the structured AI Wellness or Pitch Room process.
18. Additional Acknowledgments (as applicable)
- I understand that Platinum Mondays training participation may be required and that program adoption is expected within 90 days of onboarding (where applicable).
- I acknowledge that facility training at the Center of Excellence requires advance scheduling (where applicable).
- I understand that interviews with Dr. Kwok and leadership are educational and promotional only, and do not constitute personal medical advice.
- I consent to participation in special recognitions (e.g., The Global Think Tank of Forward Thinkers – Angels of Alignment) if selected, understanding selection is not guaranteed.
19. Final Consent
By submitting a registration form, application, or payment related to AI Wellness programs, events, media, or ecosystem activities, I confirm that I have read, understood, and agreed to abide by the above Agreements, Disclaimers, Consents & Code of Conduct as a condition of my participation.
